We provide here a pdf file of the cases we have researched, dividing them between cases where we had interviews with the people concerned and those where we relied entirely on publicly available data. In some cases, such as that of Hurricane Katrina and Blair’s decision to go to war in Iraq, the publicly available information is very extensive.

Each case has then been scored against the four main causes of decision errors

  • “Major” means the factor was probably a major cause.
  • “Some” means the factor probably had some influence.
  • “Limited” means the factor probably had some, but very limited, influence.
  • “?” means we do not have enough information to make a judgment.
  • “-” means the factor probably had no influence
  • Where significant other influences were at work, these are listed in the “Comments and other” column
